Extreme Pain IBSD

Hi 

Anyone else get extreme pain, to the point of being sick or feeling faint with IBS-D? 
 

I have regular colonoscopies and done tests to rule out celiac disease, IBD etc. 

 

Recently during my flare ups it’s just such extreme pain. Nothing I do can ease the pain. I do get relief as soon as a bowel movement has past but it’s just builds straight back up again. 
 

I feel like my default is to reach for Imodium but I would love to address why the pain is so extreme initially.

Hi Bay,

I have had extreme pain similar to how you have described it. This could be caused by a host of things so it's good that you are being scoped to rule things out.

Has anyone ever suggested trying a low dose of a neuromodulator medication (formerly called antidepressants)? In low doses these could dull the pain, but it takes a little while to feel the effects of the medication.

I use a low dose of amitriptyline (10-20mg) daily and this helps lessen the pain. The added side effect is that it also tends to cause constipation.

Yes have had the similar problems before just two weeks ago stared at 8:00 am ever 30 minutes on the toilet felt like I was trying to birth my bowels out and the cramping so bad thought I was going to vomit never let up till around 4:00 pm wish there was something that would stop these attacks they are the worst

There are 2 new classes of medications that are being added to the mix for treating abdominal pain namely Azapirone and Atypical antipsychotics. Gabapentin and Pregabalin are also being used. It would take an experienced professional who would know how to prescribe one of these.

Aside from meds, Cognitive Behavioural Therapy (CBT) and Hypnotherapy are considered evidence-based treatments for pain.
