EGJ
Hey guys, I’m still pretty newly diagnosed, i‘m 19 and female for reference. The low fodmap diet has been effective for me and im now in the reintroduction stage. So far i’ve identified gluten and onions as problem foods for me. However, I was wondering if a fast reaction to problem foods is normal and if so then why? My IBS_D usually kicks in within 20 minutes of consuming these foods with cramps and diarrhea. I was wondering how the problem foods could possibly trigger my stomach so quickly as I thought foods were supposed to take a while to suggest however my reactions are immediate. Any help with understanding this would be greatly appreciated thank you!

Much of the problem is that whenever we eat it tells our colon to empty out to make room for more food/waste (gastrocolic reflex). This is normal; however, in people with IBS it is greatly amplified and is referred to as visceral hypersensitivity. This causes the feeling of cramps and diarrhea in some. People with constipation have the same issue, except they might have the abdominal pain with no bowel movement. So, you are right. The food that you just ate aren't the culprit. It's the strong gastrocolic reflex. It normally takes about 72-hours for food that you just ate to poop it out. The way to control this can be via medication or training with cognitive behavioural therapy or hypnotherapy. Some people use an antispasmodic medication before meals to reduce the reflex. I hope this is helpful info.
